NeighborLink Fort Wayne Foundation (“NeighborLink”) is committed to the safety and well-being of all participants and volunteers. In consideration for the opportunity to participate in Camp NeighborLink, to be held July 10–11, 2025, the undersigned organization, National Young Leaders Institute (NYLI), agrees to the following terms and conditions on behalf of its staff and youth participants.

PARTICIPATION AGREEMENT AND WAIVER OF LIABILITY

Assumption of Risk:

National Young Leaders Institute (NYLI) understands and acknowledges that participation in Camp NeighborLink activities, including transportation to and from such activities, carries inherent risks. These risks may include, but are not limited to, personal injury, property damage, sickness, exposure to communicable and infectious diseases (including but not limited to COVID-19), and other potential hazards.

Release of Liability:

National Young Leaders Institute (NYLI), on behalf of itself, its staff, volunteers, and youth participants, voluntarily assumes all risks associated with participation in Camp NeighborLink and hereby releases, waives, discharges, and covenants not to sue NeighborLink Fort Wayne Foundation, its officers, directors, employees, agents, volunteers, and representatives (collectively, the “Released Parties”) from any and all claims, demands, causes of action, losses, or liabilities of any kind or nature, whether known or unknown, arising out of or related to participation in Camp NeighborLink.

Indemnification:

National Young Leaders Institute (NYLI) agrees to indemnify, defend, and hold harmless the Released Parties from any and all liability, loss, damage, or expense, including reasonable attorney’s fees, incurred as a result of any claims, demands, or causes of action brought against the Released Parties related to the participation of National Young Leaders Institute (NYLI) staff and youth in Camp NeighborLink.

Medical Treatment Authorization:

National Young Leaders Institute (NYLI) authorizes NeighborLink and its representatives to secure necessary medical treatment for any of its staff or youth participants in the event of illness or injury and accepts full financial responsibility for such treatment.

ACKNOWLEDGEMENT AND SIGNATURE

By signing below, I affirm that I am an authorized representative of National Young Leaders Institute (NYLI) and have the authority to execute this Agreement on behalf of the organization. I have carefully read and understand this Agreement, and I voluntarily sign it on behalf of the organization with full knowledge of its legal consequences.
